The Federal Aviation Administration has published a special airworthiness information bulletin (SAIB) involving the cabin door for several models of Schweizer helicopters. The following models are listed in the bulletin: 269A and A-1, 269B, TH-55A, 269C s/n 001 through 1917, 269C s/n 0001 through 347 and 269D s/n 0001 through 0073. The agency is recommending for all operators to implement actions outlined in Schweizer service bulletins DB-026, C1B-028 and B-292, as well as conduct a visual check of the cabin door hinge installation before each flight, until service bulletin work is completed at the next maintenance check. According to the FAA, since 2005 there have been several instances of the cabin door coming off due to serrated hinge pins separating from the door hinge pins. The Schweizer service bulletins instructed maintainers on how to weld a stainless steel rod on the ends of the hinge pins receptacles. There have been no reported incidents of the problem since December 2007.
